United Abilities Network provides person-centered, 24/7 Individualized Supported Living services for children, youth, and adults with developmental and intellectual disabilities throughout Missouri. Our ISL services are designed to help individuals live safely, confidently, and meaningfully in less restrictive, more inclusive home and community settings.
 

UAN has a special focus on supporting individuals with high behavioral support needs. We work closely with families, guardians, case managers, nursing professionals, and behavioral providers to create individualized supports that promote independence, stability, dignity, and quality of life.

Individualized Supported Living (ISL)

UAN provides training, modeling, and encouragement to help individuals develop important life skills. Areas of support may include self-care, communication, social skills, decision-making, self-advocacy, responsibility, and daily problem-solving.

UAN collaborates with Applied Behavior Analysis providers and other behavioral professionals to help implement individualized Behavior Support Plans. These plans are designed to improve behavior, communication, social skills, emotional regulation, and overall quality of life.


 

UAN assists individuals with nutrition, wellness, and fitness goals. Support may include planning and cooking nutritious meals, encouraging physical activity, attending wellness appointments, and building healthy routines.

UAN uses positive, person-centered strategies to help staff respond effectively to challenging behaviors. The agency will adopt the Positive Behavior Support Curriculum: Tools of Choice to equip staff with practical skills that promote safety, respect, and long-term success.

Self-Advocacy and Personal Choice

UAN encourages individuals to make choices, express preferences, and take an active role in their lives. We support each person in building confidence, understanding their rights, and developing their own voice.

UAN provides transportation for activities, appointments, work, and community participation using UAN vehicles when available. We also assist individuals with accessing OATS Bus, paratransit, and public transportation to increase independence and community access.

UAN assists individuals with the everyday skills and routines needed to live as independently as possible. Support may include meal preparation, housekeeping, laundry, personal hygiene, medication administration, money management, and other daily living needs based on each person’s support plan.

Community Resource Access


 

UAN helps individuals and guardians connect with community resources that support independence, safety, health, employment, recreation, and personal growth.
 

When needed, UAN works with Targeted Case Management agencies to request referrals to the DD-Psychiatric Stabilization Service Pilot. This support helps prevent placement disruption and may reduce psychiatric hospitalizations, emergency room visits, law enforcement involvement, self-injurious behaviors, and other crisis situations.

Individualized Assistance


 

Every person’s needs and goals are different. UAN provides additional assistance as desired by the individual and/or guardian, based on the person-centered plan and approved services.
 

UAN supports individuals as they explore their individuality, relationships, personal boundaries, and sexuality in a respectful and appropriate way. Support is guided by each person’s needs, goals, guardian involvement when applicable, and individual service plan.
